When you join our dental practice,you will benefit from our exclusive Continuing Care Dental Warranty. We are proud of our services and the benefit they provide for our clients. Our goal is to have the work we do last for many years and to exceed your expectations. We are able to offer you this peace of mind because we have high confidence in our materials and workmanship. However, the primary key to your long-term success is how well you perform your care at home: Brushing, flossing, using fluoride and other recommended products. Another key to success is regular professional exams, cleanings, x-ray films and fluoride.

With this in mind, we are pleased to offer the following exclusive Dental Warranty Protection Policy for:

• All Fillings for 2 full years from date of their placement
• All Porcelain Crowns, Bridges, Inlays, Onlays, Veneers for 5 full years from date of their placement

If these restorations break with normal use during this period, we will replace or repair these free of charge.

This warranty will be valid provided that you :

1. Maintain continuous, uninterrupted membership in our practice
2. Have professional cleanings. polishing and fluoride application as recommended by your dentist and hygienist (at least twice per year) with no appointment varying more than 15 days of your recommended schedule
3. Use an electric toothbrush and dental floss every day
4. Agree to wear any protective night guard as recommended, for clenching or grinding
5. Have the repair/replacement done @ our practice
